I-907 Guidelines for Completion

Main content start

Part 1

Item 1: If you’ve been given an A-number, put it here. Most nonimmigrants do not have an alien registration number, which is mostly confined to use by those have filed for permanent residency. If for some reason you have an A number and know it, include it here. CIS assigns an A-number to OPT applicants; you can find it on your card.

Item 2: You probably do not have a USCIS Online Account Number. If you do, you should consider filing your forms electronically, if you are eligible to do so. You can check your eligibility to file your underlying benefit request by visiting the USCIS website. You can check your eligibility to form I-539 online, specifically, by visiting the USCIS website. Please note that you can only file form I-539 online if you are applying only for yourself, without co-applicants (without form I-539A). Please check the above website just prior to filing to understand if rules/terms have changed.

Item 3: Put your name information in the respective fields.

Item 4: Leave this blank.

Item 5: Put your mailing address in the fields provided. You must be able to receive mail at this address until your application is decided and CIS can mail you the decision. This could take somewhat longer than the 30 days needed for Premium Processing due to the additional time it takes USCIS to produce and mail the approval, among other possible factors.

Item 6: Mark the appropriate box.

Item 7: If you reside at an address other than the one where you receive mail, insert the requested information regarding where you are physically living in the fields provided.

Item 8: Mark, you are the applicant (box 3).

Part 2

Item 1: You’re filing form I-539.

Item 2: If you don’t have a receipt number at this point, add “N/A” in this field; if you have an application pending list the application receipt number located in the upper left hand corner of form I-797 notice of action.

Item 3: If you’re filing this form I-907 with a DS-2019, mark “J-1” in this box, as you’re seeking J-1 status. If you’re filing with an I-20, mark “F-1” in this box, as you’re seeking F-1 status.

Item 4: Put your name information in the fields provided as you are the applicant.

Item 5: Put your name information in the fields provided.

Item 6: Leave these fields blank.

Item 7: Leave this field blank.

Item 8: Put “As in Part 1.”

Part 3

Item 1: Mark Box A, “I can read and understand English…”.

Item 2: Do not mark the Box.

Item 3 – 6: Complete the boxes with your personal information. You don’t have to provide a fax number or more than one phone number. Providing an email address is strongly recommended so you can get electronic case updates via email.

Item 7: Sign and date the form in the fields provided.

Parts 4 & 5

You can leave the Interpreter and Preparer fields blank. A preparer is a third party who completes the form for you, so isn’t relevant to your application. You do not need to sign a second time in these sections.

Part 6

This part of the form gives you the opportunity to explain any answers you feel need more context. Be sure to put your name and A-number, if any, in the fields provided. Note the page, Part and Item numbers of the answers you want to explain, and use the text section to provide the information.
